a. First complete the form;
b. Second print two copies of the form keeping one copy for your records and another copy for you to send with your check for annual dues;
c. Third use the submit button to send an electronic copy to NCPCU headquarters.

$100 dues for assets less than $10 Million
$250 dues for assets $10 - $25 Million
$350 dues for assets $25 - $50 Million
$450 dues for assets $50 - $75 Million
$550 dues for assets $75 - $100 Million
$1,000 dues for assets over $100 Million
