The Credit Union hereby accepts and agrees to the terms and conditions of the National Council of Postal Credit Union’s Emergency Cash Network Master Agreement and agrees to become a participating credit union there under. Execution of this Affiliation Agreement acknowledges receipt of a copy of the said Master Agreement. This credit union agrees to honor all Emergency Cash Network Funds Advance Vouchers executed by members of this credit union and presented for payment and also to act as a Disbursing Credit Union for members of other participating credit unions all pursuant to the terms of the Master Agreement.

On this day of , , before me, personally cameto me personally known, who being by me duly sworn, did depose and say that he/she resides at ; that he/she is the President/Chairman of the Board of CREDIT UNION, the corporation described in and which executed the foregoing instrument; that he/she knows the seal of said corporation; that the seal affixed to said instrument is such corporate seal; that is was so affixed by order of the Board of Directors of said corporation/ and that he/she signed his/her name thereto by like order.
